In recent years, asymptomatic virus carriers have turned into a global health phenomenon, especially in pandemics. These are individuals who, even though infected with the virus, do not show any signs of the virus. Such carriers contribute greatly to the dissemination of contagious diseases, especially those affecting the respiratory system. Due to the further effects of COVID-19, asymptomatic carriers have become one of the worst nightmares in the world, yet the problem extends way beyond this coronavirus.
One such is human metapneumovirus. It is a viral infection like flu, bringing symptoms of fever, coughing, wheezing, amongst other related cases. The infection mainly affects small kids, old persons, and those individuals who are suffering from several illnesses, since it’s this kind of group of people that develop grave diseases. While many of the recent reports on HMPV infections are demonstrating a decline, particularly in the northern parts of China, they do so partly because more sensitive diagnostic testing has improved upon reporting. Even though the number of cases may be decreasing, the risks associated with this silent transmission are significant for those who are at higher risk.
According to Wang Liping, a leading expert at the Chinese CDC, this virus has been circulated in the population for decades and infects most children by the time they are five years old. It could even be responsible for cold-like symptoms. A problem is that a large proportion of the population infected with HMPV might not have developed symptoms; therefore, individuals can spread viruses to others without knowing. That’s a big deal, public health-wise, as asymptomatic carriers can spread the virus to the most vulnerable people-those with compromised immune systems or chronic conditions.
Asymptomatic carriers have been one of the main factors in how fast the COVID-19 pandemic has spread. According to a 2020 study published in The Lancet, asymptomatic individuals may have accounted for as many as 40% of COVID-19 transmissions worldwide. This realization shifted the paradigm of public health measures from the simple isolation of symptomatic individuals to the silent spreaders who may remain unaware of their infection status. Thus, early identification and monitoring of asymptomatic virus carriers became an integral link in the chain of efforts at controlling the virus.
This is, by no means, exclusive to COVID-19. According to health experts, asymptomatic carriers could also play a very important role in other respiratory pandemics. In the case of HMPV, although the WHO has not reported any unusual outbreaks of the virus, asymptomatic cases have no doubt helped the virus to continue circulating. The winter time is when respiratory illnesses hit their peak and hospitals are filled with patients all over the world; thus, the need for early detection and early intervention is highly important.
Complicating this matter, for most respiratory viruses, including HMPV, there are no vaccines or specific antiviral treatments available. Whereas influenza and COVID-19 have vaccines developed for them, there are no vaccines for HMPV. Lack of such medical interventions accentuates the need for adherence to other non-pharmaceutical interventions containing spread by maintaining social distancing, frequent hand-washing, and wear masks at crowded places. Public health hygiene campaigns with an emphasis on asymptomatic risk will go a long way in controlling these viruses outbreaks.
Recently, there has been an increased demand for updated data on the identification and follow-up of asymptomatic carriers. Increasingly sensitive diagnostic testing, including PCR and antigen testing, has facilitated the improved identification of respiratory viruses, including HMPV. Thus, infections that were previously not noticed can now be detected. However, even from the data presented by the Chinese CDC, surveillance is still incomplete, most especially in areas with scarce health resources, particularly rural areas.
Other than the advances in diagnosis, scientists work on vaccines against these viruses. Though there is no vaccine currently available for HMPV, several are in development and may be improved with advances in mRNA technology, similar to COVID-19 vaccines. Translating lessons learned during the pandemic, researchers are working toward more potent vaccines for a wider array of respiratory viruses, including HMPV. One of the big challenges to controlling pandemics and halting the spread of respiratory diseases is asymptomatic spreaders, those people who, although they do not show symptoms, nevertheless can carry and spread an infection.
As long as the world continues to grapple with these evolving public health threats, the asymptomatic transmission challenge must be met head-on through various strategies, including enhanced surveillance, rapid methods of detection, public health education, and developing vaccines and treatments capable of stopping their spread. It is, therefore, of importance that the global health community continues to protect public health vigilantly and contains the spread of infectious diseases as new challenges continue to emerge.
In conclusion, the experience obtained in the management of the COVID-19 pandemic and recent data on HMPV infections in northern China bring into sharp focus the continuing threat from asymptomatic carriers. While the situation has considerably improved regarding methods of detection and surveillance, the global health community should be adequately prepared for new and existing viruses, emphasizing the prevention of asymptomatic spread. Lessons learned during this pandemic will go a long way to help in any future health crises.
